The Community Vital Signs (Vital Signs) Initiative is proud to announce the launch of its newly redesigned website. The website now clearly defines Vital Signs’ purpose and what it seeks to accomplish. The Vital Signs website is more functional, vibrant, and impactful than before.

In 2015, Vital Signs launched the Community Transformation Plan which served as the county’s health improvement plan to implement goals and strategies that community members identified as priorities.  This community-driven effort supports the Countywide Vision’s Wellness element. The website will allow residents to be better informed on the status of the events and progress on implementing the  Community Transformation Plan.

The redesign, which further reflects the vibrancy of San Bernardino County and its residents, is now easier to navigate, with improved functionality across all platforms and browsers. Visitors will experience a better user interface with intuitive streamlined menu operations that will direct visitors to community efforts, data, and information to keep them informed of the progress and the efforts of the initiative.

New features include:

- Defined priority areas, goals, and strategies – to support the  Community Transformation Plan

- Access to Data Portal – to measure and track performance and to provide perspective

- Interactive Community Events Calendar – to engage and mobilize community support and action

- Better defined Quick Links – to enhance learning and to support knowledge acquirement

There are several entry points allowing residents to participate in helping make San Bernardino County a better place to live, work, and play. Users can begin by visiting the website at www.CommunityVitalSigns.org and take advantage of the site’s new features and benefits.

The goal of Community Vital Signs is to improve the health and wellness of county residents through a health improvement framework. This is achieved through a collaborative effort, using evidence-based goals and priorities that align resources to empower residents to live healthy lives. Vital Signs, alongside county residents and partner organizations, strives to create a county of wellness where everyone has an equal opportunity to make healthy choices no matter their circumstances.
